 Immunization Requirements

PRESCHOOL
  • DTP (Diptheria, Pertussis, Tetanus)
    Series of 4 doses
  • Polio (Trivalent, oral or inactivated)
    Series of 3 doses
  • MMR (Measles, Mumps, Rubella)
    one dose on or after the first birthday
  • Hib (Haemophilus influenza type b)
    per immunization schedule
  • Varicella (Chicken Pox)
    one dose on or after the first birthday
  • Hepatitis B
    Series of 3 doses, appropriately spaced
  • Lead Screening for children under 6 yrs
K - GRADE 5
  • DTP (Diptheria, Pertussis, Tetanus)
    4 or more doses with the last dose on or after the 4th birthday
  • Polio (Trivalent, oral or inactivated)
    3 or more doses with the last dose on or after the 4th birthday
  • MMR (Measles, Mumps, Rubella)
    One dose on or after the first birthday with a Measles booster no less than 28 days later.
  • Varicella (Chicken Pox)
    One dose on or after the first birthday for children in kindergarten through 8th grade
GRADES 6 - 8
  • DTP (Diptheria, Pertussis, Tetanus)
    3 or more doses with the last dose on or after the 4th birthday
  • Tdap
    6th & 7th graders - one dose regardless of the interval since the last DTaP, DT or Td dose, 8th graders who have not already received Tdap are required to receive 1 dose regardless of the interval since the last DTap, DT or Td dose.
  • Polio (Trivalent, Oral or Inactivated)
    3 or more doses (same type of Polio vaccine) with the last dose on or after the 4th birthday
  • MMR (Measles, Mumps, Rubella)
    One dose on or after the first birthday with a Measles booster no less than 28 days later.
  • Hepatitis B
    Series of 3 doses, appropriately spaced
  • Varicella (Chicken Pox)
GRADES 9 - 12
  • DTP (Diptheria, Pertussis, Tetanus)
    3 or more doses with a booster on or after the 4th birthday and a booster every 10 years
  • Tdap
    9th & 10th graders - one dose regardless of the interval since the last DTaP, DT or Td dose, 11th & 12th graders who have not already received Tdap are required to receive 1 dose regardless of the interval since the last DTap, DT or Td.
  • Polio (Trivalent, Oral or Inactivated)
    3 or more doses (same type of Polio vaccine) with the last dose on or after the 4th birthday
  • MMR (Measles, Mumps, Rubella)
    One dose on or after the first birthday with a Measles booster no less than 28 days later
  • Hepatitis B
    Series of 3 doses, appropriately spaced
  • Varicella
    One dose on or after the first birthday for children in 9th and 10th grade.
